What did researchers find?

Among people who did not join the exercise program, those taking creatine gained roughly 2.4 pounds of lean tissue over 12 weeks. They also got stronger on the leg press and bench press, and their muscular endurance improved. The researchers also saw favorable changes in some blood fats, HbA1c (a long-term blood sugar marker) and some memory and thinking measures, but they called these results exploratory.

In the exercise and diet group, those taking creatine added nearly three pounds of lean tissue while also losing fat. People on the same program taking a placebo showed little change in lean tissue. The creatine group also had a larger drop in body fat percentage and bigger gains in strength and time to exhaustion.

Creatine did not improve aerobic fitness beyond what exercise alone achieved. The supplement was well tolerated, according to the researchers.

The study started with 73 healthy, inactive adults, and 64 finished it; their average age was 54.5. Each person first chose whether to join an exercise and weight-loss diet program or stay without exercise. Within each group, they were then randomly assigned to take creatine or a placebo every day, with neither participants nor researchers knowing who got which. The exercise program combined resistance and aerobic training three times a week, with a diet creating a modest calorie deficit. At the start, at 6 weeks and at 12 weeks, the researchers measured body composition with a DXA scan, took fasting blood samples and ran memory and thinking tests.

What does it mean for you?

Creatine may help maintain muscle and strength in middle age, when keeping muscle becomes more important. But exercise still had the bigger effect: people who combined creatine with exercise improved the most, and creatine did not boost aerobic fitness beyond exercise alone.
